Results and conclusions: Two studies on the use of systemically administered antibiotics in the treatment of peri-implantitis were identified. Both studies involved a case series without controls. Five studies on locally delivered antibiotics were found. Overall, 71.7% of the 120 peri-implantitis subjects exhibited submucosal bacterial pathogens resistant in vitro to one or more of the tested antibiotics.ConclusionsPeri-implantitis patients frequently yielded submucosal bacterial pathogens resistant in vitro to individual therapeutic concentrations of clindamycin, amoxicillin, doxycycline, or metronidazole, but only rarely to both amoxicillin Background: Peri-implantitis due to infection of dental implants is a common complication that may cause significant patient morbidity.

Surgical treatment for peri-implantitis removes infected gum tissue, manually scraping away plaque and tartar buildup from around the implant. For peri-implant infections, there is scarce evidence for the use of systemic antibiotics with no proven effective treatment protocol for good results on a long-term basis. Clinical relevance Antimicrobial resistance has emerged as a serious problem worldwide and is one of the most important threats to global health as a result of antibiotic overuse. If peri-implant mucositis progresses and turns into peri-implantitis several approaches, depending on the severity of the bone loss around the implant, can be adopted. The first step in treating peri-implantitis (as in periodontitis) is professional non-surgical therapy together with the use of chlorhexidine mouthwashes and gels to decrease the number of bacterial deposits. Peri-implantitis is an inflammation of the soft tissues around a dental implant, leading to bone loss. It's a common condition, affecting between 5% to 50% of people who have dental implants, but with the right strategies, it can be prevented.
Antibiotics treatment may have to be repeated within a few weeks or months. Implant removal. If peri-implant mucositis has developed into peri-implantitis and a significant amount of bone was lost, the implant may have to be removed.

Background: This systematic review aims to assess the current evidence on the efficacy of surgical and non-surgical debridement techniques in the treatment of peri-implantitis lesions without the use of any antimicrobials. Method: Five electronic databases (MEDLINE, Pubmed, Scopus, CINAHL and Cochrane) were used, alongside hand searches, to find relevant articles. 2012-01-13
The goal of peri-implantitis treatment is to arrest the progression of the infection and maintain the implant. The most important part of treatment is to find and treat the cause of the infection.

Peri implantitis (like gum disease) progresses in stages. In the early peri implantitis stages, a person may experience swollen, tender gums and gum sensitivity. As the stages progress, the gums start to bleed, the dental implant will loosen up, and the jaw becomes tender and increasingly sensitive.
This disease is associated with loss of supporting bone, bleeding on probing, and occasionally suppuration.
